Well Child Appointment Request
You can stay connected with Mack and Poole Pediatrics 24/7 by simply filling out the form below. For your convenience, someone from our practice will call or email you back to confirm the date and time you requested or offer alternative dates and times that accommodate your schedule. We should recontact you within one business day for confirmation of your visit.
Please include in the request whether you would prefer to see Dr. Mack, Dr. Poole, or Dr Easterling. Also please list any alternative dates/times that would be acceptable to you if your initial request is not available. This request page is not to be used to schedule a visit for an acute illness, but to request a well child exam.
Sick child appointments should be made by calling our office at 859-260-7700. We answer our phones from 7:30 am to 5:00 pm, Monday through Friday and 7:45 am to 11:00 am on Saturdays.
